MyWishes Care is a specialist Advance Care Planning documentation and coordination system for heath and care professionals.
Advance Care Planning is MyWishes Care’s core and only focus. It is a low cost, high value intervention ensuring that ‘what matters most’ is documented and shared within both clinical and non-clinical settings.

MyWishes Care is ‘people centred’ ensuring that the needs of those being cared for, friends, family members and professionals are all adhered to.
We are the only care planning / EPaCCS platform that utilises the Gov.UK design system.
This makes our solution simple for care professionals who are not confident using technology and for those who do not speak English as their first language.
Our team supports organisations when transitioning away from legacy (older) and over complicated solutions.
If you are still using paper care planning forms, we will support your organisation when migrating over to digital or taking a hybrid approach between the two formats.
Easy to use interface
Our interface is purposefully simple, jargon free and easy to use.
It has been developed with the understanding that the professional supporting the patient with their Care Planning journey might not be confident with using technology or have English as their first language.
Questions are asked and answered in a logical step, by step way. Progress is saved and rich in-depth conversations can take place.
Multi-media recordings and externally created documents can be uploaded, saved and be available to reference at anytime.
The sharing of completed Care Plans is easy and intuitive. This can take place within the platform itself or when downloading, printing and emailing documents. Realtime interoperability can take place when external platforms and databases have the necessary infrastructure in place to receive relevant information (APIs etc).
Each time a care plan is shared the action is logged and shown between the care organisation’s team for transparency and accountability.
Voice to Text Transcribing (using Artificial Intelligence)
Conversations on MyWishes Care can be captured in audio, in realtime.
Our AI Assistant transcribes the audio into text whilst the person tells the care professional ‘what matters most’ to them.
This information is inserted in a text format into the relevant fields within their Advance Care Plan.
The information captured is reviewed with the person and tweaked when it is relevant to do so. It is then saved and shared.
The outcome is richer conversations and high quality documentation. Our voice to text technologies saves time and associated costs for care organisations whilst documenting Advance Care Plans.
Real-Time Progress and analytics
Realtime data is available and displayed for internal and external review.
Core analytics can be broken down into day, week, month and year charts. This allows for ongoing motivation and improvement.
We call this “actionable data”. In short, the data displayed leads to improved awareness, reflection, better team cohesion and action.
Datasets can also be used for compliance and external validation when shared with third party regulators.
Team Management
Every organisation using MyWishes Care needs at least one ‘Manager’ to be assigned at the start of the project.
Managers are responsible for onboarding colleagues and assigning the level of access each colleague should have.
Some colleagues might be assigned ‘View Only’ access. This will allow them to view Care Plans but not edit them. Others might be assigned as a full ‘Team Member’. This will allow the person to view and edit Care Plans and patient information.
When someone joins an organisation’s team or leaves, the Manager(s) are responsible for revoking their access. Revoking access does not alter or delete any of the care plans or information that the person had previously documented.

If you feel that MyWishes Care isn’t suitable, you are welcome to cancel at anytime. We won’t ask you for any debit card details before the trial to help make things simple.
