About our service

MyWishes Care is a high quality Advance Care Planning intervention for care providers.

It utilised the UK’s standard, PBRS care planning questions (2025).

It has been built in the Gov.UK design system for ease of use and regulatory compliance purposes.

The design is simplistic and minimal. This ensures that professionals who are not tech savvy and/or have English as their first language are able to utilise it effectively.

Professionals are able to create and share Care Plans internally and externally with both patients and others involved with the person’s care. Assigned team members are able to view and edit care plans when each person’s circumstances change. 

How we are different

Other care platforms shoe-horn Care Planning into their services as an afterthought. MyWishes Care takes a different approach. Our service has been specifically built for care planning and the individual needs of the person.

Our low cost, high-value intervention improves advance care planning and the positive outcomes it has for the person, their loved ones and the wider health and care ecosystems.

Case Loads

Everyone within a care organisation’s ‘Team’ can see and/or edit patient / resident details.

Tasks and actions are documented and logged within a digital trail. Each professional shares individual and collective accountability.

Permission settings

Every care organisation using MyWishes Care has at least one ‘Manager’ assigned to them.

It is simple and easy for each ‘Manager’ to invite colleagues to view and/or edit patient records. Each care organisation is in charge of onboarding and removing colleagues. 

Collectively, each organisation is internally accountable for the amount of care plans and the quality of the documentation. 

Training

Supporting residents at AgeUK, Islington. 

As part of our service we provide face to face training, pre-recorded training videos, over the phone and email support.

This helps each member of staff become familiarised with the MyWishes Care platform and in doing so, provides better support to the communities they serve. 

When extra support and training is required we will deliver it within minutes of the request. 

Due to the simplistic nature of MyWishes Care, most health and social care professionals will not require much support to start using and utilise the service.

About our campaigning

We campaign and provide thought leadership in areas relating to care planning and end of life planning. We work with the media, academia, policy makers and national organisations to do so.

Our work is largely focused within the UK however we have also worked with international organisations such as the European Law Institute and the European Association of Palliative Care.
