Advance Care Planning supports people to live with comfort, dignity, and care that truly reflects their wishes. It is important that hospices support society with advance care planning both in the community and when someone is admitted to the Inpatient Unit.

What is Advance Care Planning?

Advance Care Planning is a thoughtful process that helps individuals consider, discuss, and record what matters most to them in their care and treatment. It ensures that, if there comes a time when they are unable to communicate or make decisions, their values and preferences continue to guide their care.

In hospice settings, where care focuses on comfort, quality of life, and emotional support, Advance Care Planning is an essential part of person-centred care. It enables the hospice team to provide care that honours the individual’s values, beliefs, and choices.

When Should Advance Care Plans Be Created?

Advance Care Planning is an ongoing and collaborative process. In hospices, it is important that every person has the opportunity to create or review their plan at a time that feels right for them. Planning may take place:

  • When the person is first referred or first admitted to hospice care

  • Whenever there is a change in their condition or care needs

  • As part of regular care reviews and conversations

  • Whenever the person or their loved ones wish to revisit their plan

Advance Care Plans should be reviewed regularly to ensure they remain meaningful, accurate, and reflective of the person’s current wishes.

£8 per resident or patient per month

Our pricing model is simple, £8 per resident or patient supported each month.

There isn’t a minimum spend and we do not charge a setup fee.
