About MyWishes Care

MyWishes Care is a specialist Electronic Palliative Care Coordination System for heath and care professionals. 

Care Planning is MyWishes Care’s only focus. It is a low cost, high value intervention ensuring that ‘what matters most’ is documented and shared within clinical and non-clinical settings.

It has been built using GOV.UK’s design system making it simple for care professionals who are not confident using technology and for those who do not speak English as their first language.

People-Centred Technology

In order for Care Planning conversations and documentation to be of high quality, the solution used should not and must not be the  professional’s main focus.

MyWishes Care is ‘people centred’ ensuring that the needs of those being cared for, friends, family members and professionals are all adhered to.

Through the utilisation of GOV.UK’s design system, care planning standards and innovation, MyWishes Care offers a unique and fresh approach to Care Planning and Care Coordination. 

Our team will support you and your colleagues when transitioning away from legacy (older) and less intuitive solutions. If you are still using paper care planning forms, we will support your organisation with your organisation’s migration to digital.

*MyWishes is the only clinician / professionally-led EPaCCS that utilises the Gov.UK design system.

Our interface is purposefully simple, jargon free and easy to use.

It has been developed with the understanding that the professional supporting the patient with their Care Planning journey might not be confident with using technology or have English as their first language. 

Questions are asked and answered in a logical step, by step way. Progress is saved and rich in-depth conversations can take place.

Multi-media recordings and externally created documents can be uploaded, saved and be available to reference at anytime.

The sharing of completed Care Plans is easy and intuitive. This can take place within the platform itself or by downloading, printing and emailing documents when they are completed or updated. Realtime interoperability can take place when external, receiving platforms and databases have the necessary infrastructure in place.

Each time a care plan is shared this action is logged and shown for transparency and accountability between colleagues within each care provider’s team.

Realtime data is available and displayed for internal and external review. 

Core analytics can be broken down into day, week, month and year charts. This allows for ongoing motivation and improvement.

We call this “actionable data”. In short, the data displayed leads to improved awareness, reflection, better team cohesion and action.

Datasets can also be used for compliance and external validation when shared with third party regulators.

Every organisation using MyWishes Care needs at least one ‘Manager’ to be assigned at the start of the project.

Managers are responsible for onboarding colleagues and assigning the level of access each colleague should have.

Some colleagues might be assigned ‘View Only’ access. This will allow them to view Care Plans but not edit them. Others might be assigned as a full ‘Team Member’. This will allow the person to view and edit Care Plans and patient information.

When someone joins an organisation’s team or leaves, the Manager(s) are responsible for revoking their access. Revoking access does not alter or delete any of the care plans or information that the person had previously documented.

In order to use MyWishes Care you will need to get in touch with us and request a free trial.

We will not ask you for you for any payment or your organisation’s banking details. If you find the platform of value within your care setting and whilst providing, you are welcome to continue to use the platform.

If however you feel that MyWishes Care isn’t suitable for your organisation and the communities you serve, you are welcome to cancel at anytime.

Pricing

£8 per resident / patient per month

Our pricing model is simple, £8 per resident or patient supported each month.

There isn’t a minimum spend and we do not charge a setup fee or training fees.
